I think I know what you're talking about in regards to having fantasies "in the moment" and then having them disappear after you cum. For many guys this correlates with the refractory period (a recovery phase post-orgasm during which it is physiologically impossible and painful for a person to experience continued arousal or additional orgasms). To get even more clinical, reaching orgasm increases the levels of prolactin, which repress the arousal properties of dopamine, which is responsible for sexual arousal.
